This simple A4 feedback sheet contains the following questions:

• Who would you say took leadership of the group?
• If it was you, why do you think you took the lead? If it was someone other than you, why did you allow them to take the lead?
• How were decisions made in the group?
• Do you feel your skills were employed to best advantage in the group? If they weren?t, why was this?
• Do you feel the group communicated effectively? If so how? If not, why not?
• Did you achieve all that you set out to achieve as a group? If not why not?
• What would you improve on next time? (Both in terms of activity and team organisation.)
